The aging process in the elderly triggers a decline in physiological functions, impacting Body Mass Index (BMI) imbalances and surges in blood pressure. This program aims to analyze the BMI and blood pressure profiles of the elderly as a basis for formulating adaptive physical exercise recommendations. A quantitative descriptive activity design with a cross-sectional approach was used in this program. A total of 50 elderly individuals at the Ngasinan Village Posyandu became target partners selected using a total sampling technique. Measured variables included BMI and blood pressure, assessed using digital scales, microtoise, and sphygmomanometers. Data analysis was carried out using percentage descriptive statistics. The results showed that the elderly population experienced a double burden of malnutrition, in which only 36% of the elderly had a normal BMI, while the remainder were dominated by obesity (36%), underweight (18%), and overweight (10%). Excess BMI potentially increases mechanical stress on joints, while the underweight category risks reducing muscle mass (sarcopenia) and triggering fall risks. In blood pressure parameters, there was a dominance of hypertension (52%) and pre-hypertension (36%), with only 12% of the elderly in the normal category. According to literature, the tendency toward increased blood pressure in the elderly can be influenced by arterial stiffness (arteriosclerosis) and body fat accumulation. Based on these findings, light-to-moderate intensity low-impact elderly exercise parameters are recommended, focusing on joint-friendly movements, breathing without holding breath, and balance training. It is recommended for posyandu cadres and sports assistants to implement physical exercises tailored to the clinical risks of BMI and blood pressure in the elderly.
